Community pharmacy support for Health Care Home patients
August 16, 2019
Pharmacies located close to participating Health Care Home (HCH) practices/Aboriginal Community Controlled Health Care Services (ACCHS) are strongly encouraged to meet with the practice staff to discuss how your pharmacy can support their HCH patients through the Community Pharmacy in Health Care Homes Trial Program.
The enrolment period for the broader Health Care Home program closed on 30 June 2019. Over 10,000 patients are enrolled across approximately 130 Health Care Home practices. The Health Care Home Program is active in 10 Primary Health Network (PHN) regions across Australia.
Now that the enrolment phase for the broader Health Care Homes Program has concluded it is anticipated that participating HCH practices will shift their focus to the broader ‘health care neighbourhood’ integrating community pharmacies and allied health into care teams with the incorporation of Medication Management Plans into patients’ shared care plans. All Health Care Homes patients are eligible to participate in the Community Pharmacy in Health Care Homes Trial Program following a referral in their shared care plan.
The current list of the participating Health Care Homes practices is available on the Health Care Homes section of the Department of Health website located at https://www1.health.gov.au/internet/main/publishing.nsf/Content/health-care-homes-professional
Right now is the time for you to contact your nearest HCH practices to discuss how your pharmacy can support their HCH patients.
In alignment with the broader Health Care Homes Program, the Government has confirmed that the Community Pharmacy in Health Care Homes Trial will be extended through to June 2021. More information on the extension will be provided as it becomes available.
The Guild is committed to the success of this trial and will continue to work closely with the 10 Health Care Homes Primary Health Networks (PHNs) to ensure that all Health Care Homes practices are aware of the significant patient and practice benefits on offer through the Community Pharmacy in Health Care Homes Trial.
