From hospital to community: a multidisplinary “continuity of care” model for cardiovascular patients involving community pharmacists

The aim of this project is to evaluate a pharmacist-delivered “continuity of care” model for cardiovascular patients returning to the community after a hospitalisation. A comparative intervention design will be used. Patients recruited from the cardiovascular ward of one hospital will receive an intervention involving a domiciliary medication management review (DMMR) shortly after discharge. This will be followed by multidisciplinary case conference and care planning meetings as necessary. Patients recruited from a second hospital will be followed as a comparator. The emphasis of the intervention is to improve communication between health care professionals both within and between hospital and community sectors and improve patient outcomes.
